Seeking an experienced Home Health Registered Nurse to deliver compassionate and skilled care to patients in their homes. This contract-based opportunity is ideal for a nursing professional who thrives in an autonomous, patient-focused environment and values effective communication, organization, and high clinical standards.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Expectation of 2532 points per week (approximately 35 patient visits daily)
- Daily travel to patient homes within a designated territory (with mileage reimbursement)
Key Responsibilities:
- Complete OASIS assessments and develop customized care plans tailored to patient needs
- Deliver skilled nursing interventions such as wound care, medication management, IV therapy, and chronic condition support
- Monitor patient conditions and promptly report any changes to the care team and physicians
- Provide clear and supportive education to patients and their families regarding illnesses, treatment plans, and medications
- Maintain thorough, timely documentation using Home Care Home Base (HCHB)
- Coordinate with an interdisciplinary team, including therapists, social workers, and aides, to facilitate comprehensive care
- Attend case conferences, staff meetings, and ongoing training to uphold best practices
- Adhere strictly to regulatory standards and agency guidelines
Qualifications:
- Active RN license in good standing
- At least 1 year of recent home health nursing experience (field experience strongly preferred)
- Demonstrated proficiency with HCHB electronic documentation system and OASIS tool
- Strong assessment, critical thinking, and time management skills
- Reliable transportation, valid drivers license, and auto insurance required
- BSN and experience managing 2532 points per week preferred
- Familiarity with Medicare and home health compliance standards
