As a Home Health Registered Nurse (RN), you will play a key role in providing high-quality, individualized nursing services through every stage of recovery and wellness. This field-based opportunity is ideal for self-directed clinicians who thrive in autonomous environments and excel at balancing clinical excellence with clear, empathetic communication.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Caseload: 2532 points per week (typically 35 patient visits daily)
- Daily travel within your assigned territory (mileage reimbursement provided)
Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursingincluding wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, recognize changes in condition, and communicate promptly with providers and team members.
- Educate patients and families about diagnoses, medications, and care plans to foster engagement and recovery.
- Document care accurately and efficiently using Home Care Home Base (HCHB).
- Collaborate with PTs, OTs, medical social workers, and aides to ensure coordinated, patient-centered care.
- Attend case conferences, staff meetings, and participate in ongoing professional education.
- Uphold agency and regulatory guidelines for quality assurance and compliance.
Qualifications:
- Current RN license (in good standing) in the state of practice
- Minimum of 1 year recent home health RN experience (field-based experience preferred)
- Proficiency in Home Care Home Base (HCHB) required
- Hands-on OASIS experience
- Strong clinical assessment, communication, and organizational skills
- Valid drivers license, reliable transportation, and current auto insurance
- Ability to manage a flexible, independent schedule
- BSN and prior experience managing 2532 points/week preferred
- Familiarity with Medicare and home health compliance standards ideal
