Job Description

An experienced Home Health Registered Nurse (RN) is needed to deliver skilled nursing care directly within patients homes, emphasizing high-quality, patient-centered services. This contract role involves conducting thorough assessments, creating personalized care plans, and working collaboratively with interdisciplinary teams to drive positive health outcomes. The position requires a self-motivated, organized nurse who thrives in an independent, field-based setting while maintaining clinical excellence and clear patient communication.

Key Responsibilities:

  • Perform comprehensive OASIS assessments and develop individualized care plans.
  • Administer skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, recognize changes in conditions, and communicate promptly with physicians and care teams.
  • Educate patients and families on disease management, treatments, medications, and preventive care.
  • Maintain precise and timely documentation using Home Care Home Base (HCHB).
  • Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides to ensure holistic patient care.
  • Participate actively in case conferences, staff meetings, and ongoing education.
  • Comply with all regulatory standards and agency protocols for documentation and care delivery.

Qualifications:

  • Active RN license in good standing within the state of practice.
  • Minimum 1 year of recent home health nursing experience with proficiency in HCHB and OASIS.
  • Strong clinical assessment and critical thinking abilities.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to manage a flexible, independent schedule with excellent communication and organizational skills. Preferred:
  • BSN degree.
  • Experience managing 25-32 patient points weekly.
  • Familiarity with Medicare and home health compliance.

Schedule and Location:

  • Full-time, 13-week contract.
  • Monday through Friday, 8:00 AM to 5:00 PM.
  • Field-based care with daily travel to patient homes within an assigned territory (mileage reimbursement provided).
