Job Description

A rewarding opportunity is available for an experienced, compassionate Registered Nurse to deliver high-quality hospice care and support in a field-based environment. This role centers on home health and hospice nursing, ensuring comfort, dignity, and guidance for patients and families during end-of-life care.

Schedule and Assignment Details:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM (occasional weekend/on-call rotation may be required)
  • Approximately 45 patient visits per day
  • Daily travel to patient homes, assisted living, or nursing facilities within an assigned area (mileage reimbursement provided)

Responsibilities include:

  • Conducting thorough nursing assessments and developing individualized hospice care plans
  • Providing direct skilled nursing focused on symptom management, pain control, and patient comfort
  • Administering medications, treatments, and interventions per physician orders and hospice protocols
  • Monitoring patient status and promptly communicating changes to the medical director and interdisciplinary team
  • Educating families and caregivers on disease progression, comfort interventions, and emotional support
  • Collaborating closely with social workers, chaplains, aides, and volunteers to meet diverse patient needs
  • Accurately documenting all care in Home Care Home Base (HCHB) electronic records
  • Participating in case conferences, team meetings, and continuous quality improvement
  • Ensuring care complies with hospice policies, procedures, and all regulatory requirements

Qualifications and Experience:

  • Current, active RN license in the state of practice
  • Minimum of 1 year recent hospice nursing experience
  • Proficiency in Home Care Home Base (HCHB)
  • Excellent clinical assessment and communication skills
  • Understanding of pain management, end-of-life care, and family support
  • Reliable transportation, valid drivers license, and auto insurance
  • Ability to work independently
  • BSN preferred; experience in palliative, end-of-life, or similar care settings desirable

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
