Job Description
Provide expert nursing care to patients in their homes as a Home Health Registered Nurse (RN) serving a field-based territory. This full-time, 13-week contract role offers the opportunity to apply your knowledge in patient-centered care, clinical assessment, and care coordination while working independently within a flexible schedule from Monday through Friday, 8:00 AM to 5:00 PM.
Responsibilities include conducting comprehensive OASIS assessments and developing individualized care plans, delivering skilled nursing services such as wound care, medication administration, and IV therapy, evaluating patient progress, and educating patients and families on treatment and preventive care. You will document all care accurately using Home Care Home Base (HCHB) and collaborate with interdisciplinary teams to ensure seamless care delivery. Daily travel to patient homes within the assigned geographic area is required, with mileage reimbursement provided.
Qualifications and Desired Experience:
- Active RN license in good standing in the state of practice
- Minimum one year of recent home health experience, preferably field-based
- Proficiency with Home Care Home Base (HCHB)
- Experience performing OASIS assessments
- Strong clinical assessment, critical thinking, communication, and organizational skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently with excellent time management
- BSN preferred
- Experience managing caseloads of 2532 points per week
- Familiarity with Medicare regulations and home health compliance is a plus
