An experienced Home Health Registered Nurse (RN) is needed to provide clinical services and support patients in achieving positive health outcomes. This contract opportunity offers a fulfilling blend of autonomy, patient-centered care, and professional growth.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits per day)
- Field-based, with daily travel to patient homes (mileage reimbursement included)
Your responsibilities will include:
- Conducting comprehensive OASIS assessments and establishing individualized care plans
- Administering skilled nursing interventionswound care, IV therapy, medication management, chronic disease care
- Monitoring patient progress and communicating changes to healthcare teams and physicians
- Educating patients and family members regarding disease management, treatment plans, and preventive health
- Maintaining accurate and timely electronic documentation with Home Care Home Base (HCHB)
- Collaborating with an interdisciplinary team, including therapists, social workers, and aides
- Participating in case conferences, staff meetings, and ongoing education
- Adhering to regulatory and agency compliance standards
Qualifications:
- Active RN license (must be in good standing)
- At least 1 year of recent home health nursing experience
- Proficiency with Home Care Home Base (HCHB)
- OASIS assessment experience
- Strong clinical assessment and organizational abilities
- Reliable transportation, valid drivers license, and current auto insurance
- BSN and knowledge of Medicare compliance are preferred
