Delivering compassionate care directly to patients homes, this contract Home Health Registered Nurse opportunity empowers skilled RNs to make a lasting difference in their community. As a key member of the interdisciplinary care team, you will ensure high-quality, patient-centered care for individuals requiring home-based nursing services.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- 13-week contract duration
- Field-based role with daily travel throughout your assigned territory
- Caseload expectation: 2532 points per week (approximately 35 patient visits daily)
Responsibilities:
- Conduct thorough OASIS assessments and craft personalized care plans.
- Deliver skilled nursing interventions such as wound care, medication management, IV therapy, and chronic disease oversight.
- Monitor patient progress, quickly recognize and report condition changes, and collaborate with physicians and the healthcare team.
- Provide education and support to patients and families on disease management, medication, and preventive care.
- Accurately document all assessments, care, and communications using Home Care Home Base (HCHB).
- Participate actively in case conferences, staff meetings, and continuing education.
- Ensure compliance with all regulatory and agency patient care standards.
Qualifications:
- Active, current RN license in Kentucky and in good standing.
- Minimum 1 year of recent home health RN experience; field/home visits preferred.
- Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS.
- Excellent clinical assessment, critical thinking, and communication skills.
- Reliable personal transportation, valid drivers license, and car insurance.
- BSN preferred, as well as experience managing comparable point-based caseloads.
- Familiarity with Medicare and home health compliance standards is advantageous.
