An experienced and compassionate Registered Nurse is needed to provide skilled nursing care for home health patients on a 13-week contract. This role offers the opportunity to make a significant impact by delivering patient-centered care directly in clients homes, supporting their health and independence with clinical excellence.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based, with daily travel to patient homes
• Expected caseload of 2532 points per week (about 35 patient visits per day)

Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized patient care plans
• Deliver skilled nursing care such as wound care, IV therapy, medication administration, and chronic disease management
• Assess patient progress and communicate changes promptly to the interdisciplinary team and physicians
• Educate patients and families on care plans, medications, preventive strategies, and disease processes
• Maintain timely and accurate electronic documentation using Home Care Home Base (HCHB)
• Coordinate care with physical therapists, occupational therapists, social workers, and home health aides
• Attend case conferences, staff meetings, and ongoing training sessions
• Comply with all regulatory, documentation, and quality standards

Qualifications:

• Active RN license (in good standing in the state of practice)
• At least 1 year of recent field-based home health RN experience
• Proficiency with OASIS and Home Care Home Base (HCHB) required
• Reliable transportation, valid drivers license, and auto insurance
• Strong clinical assessment, critical thinking, and independent decision-making skills
• Excellent communication and organizational abilities
• BSN and experience managing 2532 points/week preferred
• Knowledge of Medicare and home health compliance is a plus

$1,709 - $1,807/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
