Job Description

An experienced and compassionate Home Health Registered Nurse (RN) is needed to deliver skilled nursing care in patients homes, promoting quality, continuity, and patient-centered care. This role requires a self-motivated and organized clinician comfortable working independently, with a strong commitment to clinical excellence and effective patient communication.

This position is based in Overland Park, KS, a vibrant community offering a balance of urban convenience and suburban charm. Responsibilities:

  • Complete comprehensive OASIS assessments and develop individualized care plans
  • Provide skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management
  • Monitor patient progress, detect changes in condition, and communicate with the healthcare team and physicians promptly
  • Educate patients and families on disease management, medications, treatment plans, and preventive care
  • Maintain precise and timely documentation using Home Care Home Base (HCHB) electronic system
  • Collaborate with interdisciplinary team membersincluding physical therapists, occupational therapists, medical social workers, and home health aidesto ensure coordinated care
  • Participate in case conferences, staff meetings, and continuing education activities
  • Comply with all regulatory and agency standards related to patient care and documentation

Qualifications:

  • Active RN license in good standing for the state of practice
  • Minimum 1 year of recent home health experience with preference for field-based care
  • Proficient in Home Care Home Base (HCHB) and experienced with OASIS assessments
  • Strong clinical assessment, critical thinking, and organizational skills
  • Reliable transportation with valid drivers license and auto insurance
  • Ability to work independently with a flexible, field-based schedule

Preferred:

  • Bachelor of Science in Nursing (BSN)
  • Experience managing 2532 points per week caseload
  • Knowledge of Medicare and home health compliance standards

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM
  • Field-based with daily travel to patient homes (mileage reimbursement provided)
  • Caseload of approximately 2532 points per week (35 patient visits daily)
