Job Description
An experienced and compassionate Home Health Registered Nurse (RN) is needed to deliver skilled nursing care in patients homes, promoting quality, continuity, and patient-centered care. This role requires a self-motivated and organized clinician comfortable working independently, with a strong commitment to clinical excellence and effective patient communication.

Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans
- Provide skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, detect changes in condition, and communicate with the healthcare team and physicians promptly
- Educate patients and families on disease management, medications, treatment plans, and preventive care
- Maintain precise and timely documentation using Home Care Home Base (HCHB) electronic system
- Collaborate with interdisciplinary team membersincluding physical therapists, occupational therapists, medical social workers, and home health aidesto ensure coordinated care
- Participate in case conferences, staff meetings, and continuing education activities
- Comply with all regulatory and agency standards related to patient care and documentation
Qualifications:
- Active RN license in good standing for the state of practice
- Minimum 1 year of recent home health experience with preference for field-based care
- Proficient in Home Care Home Base (HCHB) and experienced with OASIS assessments
- Strong clinical assessment, critical thinking, and organizational skills
- Reliable transportation with valid drivers license and auto insurance
- Ability to work independently with a flexible, field-based schedule
Preferred:
- Bachelor of Science in Nursing (BSN)
- Experience managing 2532 points per week caseload
- Knowledge of Medicare and home health compliance standards
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Field-based with daily travel to patient homes (mileage reimbursement provided)
- Caseload of approximately 2532 points per week (35 patient visits daily)
