A rewarding opportunity exists for an experienced Home Health Registered Nurse (RN) to deliver high-quality clinical care in patients' homes within the Overland Park, KS area. This full-time, 13-week contract role offers a balanced Monday through Friday schedule from 8:00 AM to 5:00 PM, providing hands-on nursing care that supports patient independence and promotes positive health outcomes in a community setting.
Responsibilities include conducting comprehensive OASIS assessments, developing and managing individualized care plans, and delivering skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management. You will monitor patient progress, educate patients and families, maintain accurate electronic documentation using Home Care Home Base (HCHB), and collaborate closely with interdisciplinary teams including therapists, social workers, and home health aides. Reliable transportation is essential as daily travel to patient homes within the assigned territory is required, with mileage reimbursement provided.
Key Qualifications:
- Active RN license in the state of practice in good standing
- Minimum of 1 year of recent home health experience with field work preferred
- Proficiency in Home Care Home Base (HCHB) and OASIS assessment tool
- Strong clinical assessment and critical thinking skills
- Reliable transportation, valid driver's license, and insurance
- Ability to work independently with excellent communication and organizational skills
Preferred:
- BSN degree
- Experience managing caseloads of 2532 points weekly
- Knowledge of Medicare and home health compliance standards
