Job Description
Take your nursing skills into the field as a dedicated Home Health Registered Nurse. In this contract position, youll deliver direct, high-quality patient care to individuals in their homes. You will play an impactful role in assessment, care planning, education, and interdisciplinary collaborationall centered around supporting patients independence and well-being.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Key Duties:
- Conduct thorough OASIS assessments and develop individualized care plans tailored to patient needs
- Deliver skilled nursing interventions such as wound care, IV therapy, chronic disease management, and medication administration in the home setting
- Monitor and document patient progress using the Home Care Home Base (HCHB) electronic system, ensuring accuracy and compliance
- Educate patients and families on diagnoses, treatment protocols, prevention, and self-care to optimize outcomes
- Collaborate closely with therapists, social workers, and aides for coordinated patient support
- Participate in team meetings, continuing education, and quality assurance activities
- Adhere to all clinical and regulatory standards to support best practices in care
Required Background:
- Active RN license, in good standing
- Minimum 1 year of recent field-based Home Health RN experience
- Demonstrable skills using Home Care Home Base (HCHB) and OASIS assessment tools
- Solid clinical judgment and organizational abilities
- Valid drivers license, reliable transportation, and up-to-date auto insurance
Preferred Experience:
- BSN degree
- Experience managing a caseload of 2532 points per week
- Familiarity with Medicare and home health compliance policies
