Job Description
We are searching for an experienced and compassionate Home Health Registered Nurse (RN) to help deliver top-quality care to patients in their homes through a 13-week contract assignment. This vital opportunity empowers you to provide skilled nursing services and patient education while ensuring a high standard of clinical care, documentation, and coordination with an interdisciplinary healthcare team.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active RN license in the state (in good standing)
- Minimum one year of recent home health (field) experience
- Experience with OASIS and Home Care Home Base (HCHB) electronic documentation (required)
- Strong clinical assessment, communication, and critical thinking skills
- Ability to manage 2532 points per week (typically 35 patient visits per day)
- Reliable transportation, valid drivers license, and auto insurance
- Flexibility and strong organizational skills for field-based work
- BSN and previous experience managing high caseloads preferred
- Knowledge of Medicare and home health compliance standards a plus
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans
- Provide direct skilled nursing care, including wound care, IV therapy, disease management, and medication administration
- Monitor patient progress and maintain timely, accurate documentation in HCHB
- Educate patients and families on care plans, medications, and preventive care
- Collaborate with physical therapists, occupational therapists, home health aides, and social workers
- Attend case conferences, staff meetings, and pursue continuing education
- Adhere to regulatory and clinical excellence standards
