Job Description

An exciting opportunity is available for an experienced and compassionate Registered Nurse to provide expert skilled nursing care in patients homes. As a Home Health RN, you will help maintain the highest standards of patient-centered care, working closely with interdisciplinary healthcare teams to promote positive patient outcomes and quality of life. Your ability to work independently and manage a flexible field-based schedule will be a key asset in delivering exceptional care across a diverse patient population.

Schedule & Assignment Details:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Expectation of 2532 points per week (typically 35 patient visits per day)
  • Daily field-based visitstravel within assigned territory (mileage reimbursement provided)
  • Electronic clinical documentation in Home Care Home Base (HCHB)
  • Utilize OASIS assessment tools

Primary Responsibilities:

  • Complete comprehensive OASIS assessments and develop individualized care plans
  • Deliver skilled nursing care including wound care, IV therapy, medication administration, and chronic illness management
  • Monitor patient status, promptly report condition changes, and coordinate with care teams and physicians
  • Educate patients and caregivers on treatment plans, medications, and prevention strategies
  • Maintain detailed and timely electronic documentation in HCHB
  • Collaborate with therapists, social workers, and home health aides for holistic care management
  • Participate in meetings, case conferences, and ongoing professional development

Qualifications:

  • Active RN license (in good standing in state of assignment)
  • At least 1 year of recent Home Health (field) RN experience
  • Proficiency with HCHB and OASIS required
  • Strong clinical assessment and communication skills
  • Reliable vehicle, valid drivers license, and current auto insurance
  • BSN preferred; knowledge of Medicare/home health compliance a plus
