An experienced Home Health Registered Nurse (RN) is needed to deliver skilled nursing care to patients in their homes. This role requires a self-motivated, organized professional comfortable working independently while upholding high clinical standards and patient communication. The right candidate will manage individualized care plans and collaborate with interdisciplinary teams to achieve positive patient outcomes.
Location:
Based in Oregon City, OR, this position involves daily travel within the assigned territory to patient homes. Oregon City offers a blend of scenic beauty and urban convenience with easy access to outdoor recreational activities, cultural sites, and a supportive community atmosphere.
Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Provide skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor and evaluate patient progress, promptly communicating any changes to the care team and physicians.
- Educate patients and families on disease processes, treatments, medications, and preventive care.
- Maintain accurate and timely documentation using Home Care Home Base (HCHB).
- Collaborate with therapists, social workers, and home health aides to ensure coordinated care.
- Participate in case conferences, staff meetings, and ongoing education.
- Comply with all regulatory and agency standards for documentation and patient care.
Qualifications:
- Active RN license in good standing.
- Minimum 1 year recent Home Health experience; field experience preferred.
- Proficiency with Home Care Home Base (HCHB) and OASIS required.
- Strong clinical assessment and critical thinking skills.
- Reliable transportation with valid drivers license and auto insurance.
- Ability to work independently with excellent communication and organizational skills.
Preferred:
- BSN degree.
- Experience managing 2532 points per week.
- Knowledge of Medicare and home health compliance standards.
Contract Details & Benefits:
- Full-time 13-week contract, MondayFriday, 8:00 AM5:00 PM.
- Caseload of 2532 points weekly (35 patient visits per day).
