An experienced and compassionate Registered Nurse is needed to provide direct patient care in the home health setting. This is a contract opportunity, offering the chance to make a meaningful impact on patient health and autonomy by delivering skilled nursing care in a field-based environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Position Details:

• Full-time, 13-week contract
• Daily travel to patient homes (mileage reimbursement provided)
• Patient caseload: 2532 points per week (approximately 35 visits per day)
• Documentation system: Home Care Home Base (HCHB)
• Assessment tool: OASIS

Key Responsibilities:

• Complete comprehensive OASIS assessments and create individualized care plans.
• Deliver skilled nursing services including wound care, medication management, IV therapy, and chronic disease management.
• Monitor and document patient progress, communicating changes promptly to the care team and physicians.
• Educate patients and families about disease processes, medications, and home care protocols.
• Collaborate with an interdisciplinary team to ensure coordinated, patient-centered care.
• Maintain accurate documentation in Home Care Home Base (HCHB).
• Participate in staff meetings, case conferences, and continuing education as appropriate.
• Adhere strictly to regulatory and agency standards for quality care and documentation.

Qualifications:

• Current, unrestricted RN license (state of practice).
• Minimum 1 year of recent home health experience, with field experience strongly preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS.
• Reliable transportation, valid drivers license, and auto insurance.
• Strong assessment, communication, and time management skills.
• BSN preferred and prior experience managing a 2532 point caseload is advantageous.
• Knowledge of Medicare and home health compliance standards is a plus.

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
