An opportunity is available for an experienced Home Health Registered Nurse (RN) to provide patient-centered skilled nursing care in the comfort of patients homes. This role demands a professional who is self-motivated, organized, and capable of working independently while upholding a high standard of clinical care and communication.
Key responsibilities include conducting comprehensive OASIS assessments, developing individualized care plans, and delivering skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management. The RN will monitor patient progress, identify changes in condition, and collaborate effectively with interdisciplinary healthcare teams, including therapists and social workers. Accurate and timely documentation in Home Care Home Base (HCHB) is essential, alongside adherence to all regulatory and agency standards.
Qualifications & Desired Experience:
- Active RN license in good standing in the state of practice
- Minimum of 1 year recent home health experience, preferably with fieldwork
- Proficiency with Home Care Home Base (HCHB) and OASIS assessments
- Strong clinical assessment, critical thinking, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently managing a flexible field-based schedule
- BSN preferred
- Prior management of 2532 points per week caseload preferred
- Knowledge of Medicare and home health compliance standards
Schedule & Location:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Field-based work with 35 patient visits per day, covering 2532 points per week
- Daily travel within the assigned territory with mileage reimbursement
