This dynamic contract position offers the opportunity to provide skilled nursing care directly to patients in their homes, ensuring continuity and quality in patient-centered care. The ideal candidate is self-driven, highly organized, and adept at working independently while consistently upholding exceptional clinical standards and engaging communication with patients and families.

Schedule:

• Full-time, 13-week contract
• Monday through Friday, 8:00 AM 5:00 PM

Desired Qualifications:

• Active RN license in the state of practice, in good standing
• Minimum of one year of recent home health experience, with field work preferred
• Proficiency using Home Care Home Base (HCHB) for documentation
• Experience completing OASIS assessments
• Strong clinical assessment and critical thinking abilities
• Reliable transportation, valid driver's license, and auto insurance
• Excellent interpersonal, time management, and organizational skills
• Ability to excel independently in a field-based environment

Preferred Experience:

• BSN degree
• Previous caseload of 2532 points/week
• Familiarity with Medicare and home health compliance regulations

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing interventions, such as wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress and communicate changes promptly to the care team
• Educate patients and families about diagnoses, medications, and preventive care
• Maintain accurate, timely documentation within HCHB
• Collaborate with interdisciplinary teams to ensure cohesive care delivery
• Participate in case conferences, staff meetings, and ongoing professional development
