Job Description

An experienced Home Health Registered Nurse (RN) opportunity is available to provide comprehensive, patient-centered nursing care within the comfort of patients homes. This role involves delivering skilled nursing interventions, conducting detailed assessments, and collaborating with interdisciplinary teams to optimize patient outcomes. The ideal candidate is organized, self-motivated, and excels in independent clinical decision-making while maintaining excellent patient communication.

Key Responsibilities:

  • Perform thorough OASIS assessments and develop individualized care plans.
  • Administer skilled nursing care including wound management, medication administration, IV therapy, and chronic disease care.
  • Monitor patient progress, recognize changes in condition, and communicate promptly with physicians and the healthcare team.
  • Educate patients and their families on treatment plans, medications, disease processes, and preventive care.
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB).
  • Collaborate with physical therapists, occupational therapists, social workers, and home health aides to ensure coordinated care.
  • Participate in case conferences, staff meetings, and continuing education.
  • Adhere strictly to regulatory standards and agency policies for documentation and patient care delivery.

Qualifications:

  • Active RN license in good standing within the state of practice.
  • Minimum of one year recent home health experience with field-based care preferred.
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
  • Strong clinical assessment, critical thinking, and organizational abilities.
  • Reliable transportation with valid drivers license and auto insurance.
  • Ability to manage a flexible schedule working independently in the field. Preferred:
  • Bachelor of Science in Nursing (BSN) degree.
  • Experience managing caseloads of 2532 points weekly.
  • Knowledge of Medicare guidelines and home health compliance.

Schedule and Contract Details:

  • Full-time, 13-week contract.
  • Monday through Friday, 8:00 AM 5:00 PM.
  • Typical caseload of 2532 points weekly (35 patient visits per day).
  • Daily travel between patient homes within an assigned territory, with mileage reimbursement provided.
