Deliver exceptional patient-centered care as a Home Health Registered Nurse, providing skilled nursing services to individuals in the comfort of their homes. In this contract role, you will perform comprehensive clinical assessments, develop individualized care plans, and collaborate closely with a multidisciplinary healthcare team to promote optimal patient outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments to inform detailed, individualized care plans.
• Deliver skilled nursing care, including wound care, IV therapy, chronic disease management, and medication administration.
• Monitor patient progress, document in Home Care Home Base (HCHB), and promptly communicate status changes to the care team and physicians.
• Provide clear education to patients and families about disease processes, medical treatments, medications, and preventive healthcare.
• Foster seamless care coordination with physical therapists, occupational therapists, home health aides, and medical social workers.
• Participate in interdisciplinary meetings, case conferences, and relevant continuing education.
• Ensure all care and documentation strictly adheres to regulatory and agency standards.

Qualifications:

• Active RN licensure in good standing in the state of practice.
• Minimum of 1 year recent experience in Home Health (field experience preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Strong clinical assessment, time management, and communication skills.
• Ability to work independently, managing a flexible field-based schedule and caseload of 2532 points per week.
• Reliable transportation, current drivers license, and car insurance.
• BSN preferred; knowledge of Medicare and home health compliance a plus.

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
