An opportunity is available for an experienced and compassionate Home Health Registered Nurse (RN) to make a direct impact in patient lives. In this contract position, youll deliver exceptional nursing care in a home setting, supporting individuals with personalized attention and expert clinical skills. Success in this role relies on self-motivation, strong organization, and a commitment to outstanding patient communication and outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments; create and manage individualized care plans.
• Provide skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, recognize changes in condition, and communicate promptly with the care team and providers.
• Educate patients and families on disease processes, medications, treatments, and preventive strategies.
• Maintain timely, accurate documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary professionals, including therapists, social workers, and aides, to ensure seamless care.
• Participate in team meetings, case conferences, and required training.
• Adhere to agency policies and regulatory standards for documentation and patient care.

Qualifications:

• Active RN license in good standing within the state.
• At least 1 year of recent Home Health nursing experience (field-based experience strongly preferred).
• Proficiency with HCHB and required OASIS assessment experience.
• Excellent clinical assessment, time management, and organizational skills.
• Valid drivers license, reliable transportation, and auto insurance.
• Ability to thrive independently and manage a flexible, field-based schedule.
• BSN preferred; prior experience managing caseloads of 2532 points per week and knowledge of Medicare/home health compliance is a plus.
