Qualifications:

• Active RN license in good standing.
• Minimum of one year experience in a Home Health RN role.
• Solid clinical assessment, patient education, and communication skills.
• Familiarity with electronic health records and mobile home health documentation.
• Valid drivers license and reliable transportation.
• CPR certification.

Key Responsibilities:

• Conduct comprehensive in-home patient assessments, monitor conditions, and update individualized care plans.
• Administer medications, wound care, and other prescribed treatments.
• Educate patients and families on diagnoses, medications, and home safety.
• Coordinate with physicians, physical therapists, and other clinicians to promote continuity of care.
• Accurately document observations, interventions, and patient responses in compliance with regulatory standards.
• Support patients with chronic illnesses, post-surgical recovery, or rehabilitation.
