An experienced and compassionate Registered Nurse is needed for a dynamic home health contract assignment. In this role, youll deliver skilled nursing care in patients homes, focusing on quality, continuity, and individualized, patient-centered treatment. This is an excellent opportunity for a self-motivated RN who enjoys autonomy and values high clinical standards.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (35 home visits per day)
• Field-based travel (daily mileage reimbursement)

Key Responsibilities:

• Complete comprehensive OASIS assessments and formulate individualized care plans.
• Deliver skilled nursing interventions: wound care, IV therapy, medication administration, chronic disease management, and more.
• Educate patients and families regarding diagnoses, medications, and preventive care.
• Track patient progress, address condition changes, and collaborate with physicians and interdisciplinary care teams.
• Maintain precise and prompt documentation in Home Care Home Base (HCHB).
• Participate in case conferences, staff meetings, and ongoing professional development.
• Adhere to regulatory and quality standards in home health care delivery.

Minimum Requirements:

• Active, unencumbered RN license in state of practice
• At least 1 year of recent field-based home health RN experience
• Experience using Home Care Home Base (HCHB) and OASIS assessments
• Strong critical thinking, clinical assessment, and communication skills
• Reliable transportation, valid drivers license, and proof of auto insurance
• Ability to manage a flexible, independent schedule

Preferred Qualifications:

• BSN degree
• Experience with similar caseload (2532 points per week)
• Familiarity with Medicare and home health compliance

$2,490 - $2,634/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
