Job Description
Deliver skilled, patient-centered care as a Home Health Registered Nurse, supporting individuals in their homes and positively impacting their health journey. This contract opportunity centers on delivering clinical excellence, fostering independence, and collaborating with an interdisciplinary team for optimal patient outcomes.
Schedule & Setting
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract
- Field-based role, traveling daily to patient homes (mileage reimbursement included)
- Caseload: 2532 points per week (typically 35 patient visits daily)
Core Responsibilities
- Conduct comprehensive OASIS assessments and craft individualized care plans.
- Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and disease management.
- Evaluate and monitor patient progress, recognizing and communicating changes in condition to care teams and physicians.
- Educate patients and families regarding conditions, treatment plans, medications, and preventive strategies.
- Maintain thorough, timely documentation using Home Care Home Base (HCHB).
- Collaborate with therapy, social work, and home health aide colleagues for coordinated patient care.
- Attend case conferences and participate in continuing education.
- Uphold regulatory and clinical standards for quality and compliance.
Qualifications
- Active RN license in good standing within the state of practice
- At least 1 year of recent home health nursing experience (field preferred)
- Experience with OASIS and proficiency in HCHB documentation system
- Strong clinical assessment, time management, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to function effectively and independently in a field-based setting
Preferred Background
- Bachelor of Science in Nursing (BSN)
- Experience managing 2532 points weekly
- Knowledge of Medicare and home health compliance
