Job Description
Deliver meaningful, patient-centered care as a Home Health Registered Nurse (RN) supporting clients in the community through direct clinical services. This contract role is well-suited for an independent, compassionate RN who thrives on building relationships and utilizes expert knowledge in home health standards.
Schedule & Setting:
- Monday Friday, 8:00 AM 5:00 PM
- Field-based position with daily travel to patient homes (mileage reimbursed)
- 13-week contract assignment
- Caseload of 2532 points per week, averaging 35 patient visits daily
Core Responsibilities:
- Complete comprehensive OASIS assessments to guide care planning
- Develop individualized patient care plans in collaboration with interdisciplinary teams
- Deliver skilled nursing interventions, including medication management, IV therapy, wound care, and chronic disease education
- Monitor and evaluate patient outcomes, reporting changes promptly to providers and team members
- Provide patient and family education on diagnoses, medications, and ongoing wellness strategies
- Accurately document visits and care provided in Home Care Home Base (HCHB)
- Attend case conferences and staff meetings, upholding regulatory and agency requirements
Qualifications:
- Active RN license (good standing)
- At least 1 year of recent clinical experience in home health (field experience strongly preferred)
- Experience with OASIS assessments and proficiency in Home Care Home Base (HCHB) required
- Strong clinical judgment, organizational skills, and autonomy in patient care
- Reliable transportation, drivers license, and current auto insurance
- BSN preferred, as well as experience handling caseloads of 2532 points per week
