As an experienced Home Health Registered Nurse, youll play a crucial role in providing clinical excellence and compassionate support to individuals in need of skilled nursing services. This is a full-time, 13-week contract opportunity, ideal for RNs who thrive in an independent, field-based role and are committed to making a difference.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive OASIS assessments to design and implement individualized care plans.
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify changes, and collaborate closely with the healthcare team and physicians.
• Educate patients and families on diagnoses, medications, and preventive care measures.
• Document thoroughly and accurately using Home Care Home Base (HCHB).
• Work collaboratively with physical therapists, occupational therapists, social workers, and home health aides.
• Attend case conferences, staff meetings, and participate in ongoing professional development.
• Maintain compliance with agency and regulatory standards.

Required Qualifications:

• Active RN license (in good standing) in the state of practice.
• At least one year of recent, relevant home health experience (fieldwork preferred).
• Proficiency in Home Care Home Base (HCHB) and experience with OASIS assessments.
• Strong assessment, critical thinking, and organizational skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Excellent communication and time management abilities.

Preferred:

• BSN degree.
• Experience managing 2532 points per week.
• Knowledge of Medicare and home health compliance standards.
