An opportunity is available for an experienced and compassionate Registered Nurse specializing in Home Health. This contract position calls for a dedicated professional committed to providing attentive, evidence-based care within a patients home. The ideal candidate excels at fostering autonomy for patients and families, while ensuring continuity and coordination of care as part of an interdisciplinary healthcare team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive OASIS assessments and build individualized care plans for home health patients.
• Deliver hands-on skilled nursing care: wound management, IV therapy, medication administration, and chronic disease support.
• Monitor patient progress, recognize and communicate health status changes, and update physicians and clinical team members promptly.
• Provide thorough education to patients and families regarding conditions, treatments, and preventive measures.
• Accurately document all visits and interventions using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and aides to ensure integrated, high-quality care.
• Attend case conferences, staff meetings, and engage in ongoing professional development.
• Maintain compliance with clinical and regulatory documentation standards.

Qualifications:

• Active and unencumbered RN license in the state of practice.
• At least one recent year of Home Health (field-based) nursing experience required.
• Proficiency in Home Care Home Base (HCHB) and experience with OASIS.
• Excellent clinical assessment, organizational, and communication skills.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to autonomously manage patient scheduling and caseloads.
• Preferred: BSN, knowledge of Medicare/home health compliance, previous management of caseloads totaling 2532 points per week.

$1,724 - $1,824/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
