Deliver high-quality patient care as a Home Health Registered Nurse, providing expert clinical services directly in patient homes. This rewarding contract position offers a unique opportunity to work independently while making a positive impact on patient outcomes and well-being. Schedule & Details:
- Contract length: 13 weeks
- Full-time: Monday through Friday, 8:00 AM 5:00 PM
- Field-based visits to patient homes within an assigned territory
- Caseload: 2532 points per week (approximately 35 visits daily)
- Daily travel required (mileage reimbursement provided)
- Electronic documentation in Home Care Home Base (HCHB) and use of OASIS
Qualifications:
- Active RN license in the state of practice, in good standing
- Minimum 1 year of recent home health nursing experience (field work preferred)
- Proficiency in Home Care Home Base (HCHB)
- OASIS assessment experience required
- Demonstrated strong clinical assessment and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work autonomously and manage an individualized schedule
- Excellent communication, organization, and time management skills
- BSN preferred
- Knowledge of Medicare and home health compliance a plus
Responsibilities:
- Complete comprehensive OASIS assessments and create individualized care plans
- Deliver skilled nursing services: wound care, medication administration, IV therapy, chronic disease management
- Monitor patient progress and communicate changes to the multidisciplinary care team and physicians
- Educate patients and families about disease management and preventive care
- Maintain accurate, timely documentation in Home Care Home Base (HCHB)
- Collaborate with therapists, social workers, aides, and participate in meetings and case conferences
