This full-time, 13-week contract role focuses on delivering skilled nursing services and fostering positive patient outcomes within a supportive, interdisciplinary environment. You will play a crucial role in empowering patients and families, while exercising autonomy and utilizing your clinical expertise in the home health setting.
Schedule and Details:
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits daily)
- Territory: Daily travel to patient homes (mileage reimbursement provided)
- Electronic Documentation: Home Care Home Base (HCHB)
- Assessment Tool: OASIS
Core Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing interventions: wound care, IV therapy, medication administration, chronic disease management
- Assess and monitor patient progress, promptly communicating updates to the interdisciplinary team
- Educate patients and families on disease processes, medications, and plans of care
- Maintain accurate, timely documentation using HCHB
- Collaborate with therapists, social workers, and aides to ensure coordinated, quality care
- Participate in case conferences, team meetings, and ongoing education
- Uphold all regulatory and best practice standards
Qualifications:
- Active Registered Nurse license (state of practice), in good standing
- At least 1 year recent home health experience (field experience highly preferred)
- Proficient in HCHB and OASIS documentation and assessment
- Strong assessment, clinical, and communication skills
- Reliable transportation, valid drivers license, and current auto insurance required
- Comfortable working independently and managing a dynamic schedule
- BSN and prior experience managing 2532 points/week preferred
- Familiarity with Medicare and home health compliance a plus
