This full-time, 13-week contract opportunity is ideal for professionals who thrive on offering individualized patient support in a dynamic, field-based setting. You will work independently to manage patient caseloads, collaborating with an interdisciplinary team to optimize outcomes and ensure each patient receives exceptional care.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop tailored care plans.
• Provide a range of skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, recognize changes in condition, and coordinate with physicians and care team.
• Educate patients and families on diagnoses, treatment regimens, and preventive care strategies.
• Accurately document care in the Home Care Home Base (HCHB) system.
• Collaborate with therapists and social workers to deliver holistic, well-coordinated care.
• Participate in team meetings, case conferences, and additional training as required.
• Ensure strict adherence to regulatory and agency standards at all times.

Qualifications:

• Active RN license in good standing.
• Minimum 1 year of recent home health nursing experience.
• Field-based background strongly preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Excellent assessment, critical thinking, and communication abilities.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently and manage your daily schedule.

Preferred:

• BSN degree.
• Experience managing a caseload of 2532 points weekly.
• Understanding of Medicare and home health compliance standards.
