An exciting opportunity is available for a skilled and compassionate Registered Nurse to deliver vital home health care services to patients in their own environments. This contract position is ideal for clinicians who are organized, self-driven, and passionate about delivering high-quality patient care in a field-based setting.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Complete thorough OASIS assessments and develop patient-centered care plans.
• Deliver skilled nursing care such as wound management, medication administration, IV therapy, and chronic disease monitoring.
• Routinely evaluate patient progress, proactively respond to changes in condition, and communicate promptly with physicians and the interdisciplinary team.
• Educate patients and families on health conditions, medication regimens, and preventive strategies.
• Ensure timely, accurate documentation in Home Care Home Base (HCHB).
• Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides to ensure seamless, coordinated care.
• Attend case conferences, staff meetings, and participate in ongoing education.
• Adhere to regulatory and agency requirements for patient care and documentation.

Qualifications:

• Active and unrestricted RN license in the state of practice
• Minimum 1 year of recent home health experience (field preferred)
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Strong clinical assessment, organizational, and communication skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to operate independently while managing a flexible schedule

Preferred:

• BSN
• Experience managing 2532 points per week
• Familiarity with Medicare and home health compliance
