Job Description

A skilled and compassionate Home Health Registered Nurse (RN) is needed to deliver high-quality nursing care to patients within their homes in Norfolk, VA. This full-time, 13-week contract position involves independent work in the field, providing patient-centered care while maintaining clinical excellence and clear communication.

In this role, you will conduct comprehensive OASIS assessments, develop and update individualized care plans, and collaborate closely with interdisciplinary healthcare professionals. Your responsibilities will include wound care, medication administration, IV therapy, chronic disease management, patient and family education, and timely documentation using the Home Care Home Base (HCHB) system. Daily travel to patient homes within an assigned territory is required, with mileage reimbursement provided.

Key Responsibilities:

  • Perform comprehensive OASIS assessments and develop care plans tailored to patient needs.
  • Administer skilled nursing care, including wound management, medication administration, IV therapy, and chronic disease monitoring.
  • Monitor patient progress and communicate promptly with physicians and care teams regarding any changes.
  • Educate patients and families on disease management, treatments, and preventive measures.
  • Maintain accurate, timely documentation using HCHB.
  • Collaborate with therapists, social workers, and home health aides to coordinate care.
  • Participate in case conferences, staff meetings, and ongoing education.
  • Comply with regulatory and agency standards for patient care and documentation.

Qualifications:

  • Active RN license in good standing in the state of practice.
  • Minimum of 1 year of recent home health nursing experience; field experience preferred.
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessment tool.
  • Strong clinical assessment abilities and critical thinking skills.
  • Reliable transportation with valid drivers license and auto insurance.
  • Excellent communication, time management, and organizational skills.
  • Ability to work independently in a field-based setting.
  • BSN preferred.
  • Experience managing 2532 points weekly is a plus.
  • Knowledge of Medicare and home health compliance standards preferred.

Schedule & Details:

  • Monday through Friday, 8:00 AM to 5:00 PM.
  • Caseload: 2532 points per week (35 patient visits daily).
  • Field-based with daily travel within the assigned territory.
  • Mileage reimbursement included.
