An excellent opportunity is available for a skilled and compassionate Registered Nurse with a background in Home Health. This contract position is focused on providing high-quality, patient-centered care directly in patients homes. The role is designed for a nurse who values independence, clinical excellence, and strong patient relationships.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active RN license (in good standing)
- At least 1 year of recent home health nursing experience (field work preferred)
- Proficiency with Home Care Home Base (HCHB) electronic documentation
- OASIS assessment experience
- Strong clinical assessment and critical thinking skills
- Reliable transportation, valid drivers license, and current auto insurance
- Ability to independently manage a flexible, field-based schedule
- Excellent interpersonal, time management, and organizational skills
- BSN preferred
- Prior experience managing a caseload of 2532 points per week
- Understanding of Medicare and home health regulatory compliance standards
Primary Responsibilities:
- Conduct OASIS assessments, develop and revise individualized care plans
- Deliver skilled nursing services including wound care, IV therapy, and chronic disease management
- Monitor patient health status, update care teams and physicians on changes
- Provide education to patients and families regarding disease processes and home care strategies
- Ensure accurate, timely charting using HCHB
- Collaborate with an interdisciplinary healthcare team for seamless patient care
- Participate in team meetings, case conferences, and ongoing education
- Comply with all relevant regulations and agency procedures
- Travel daily to patient homes with mileage reimbursement provided

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
