Job Description
An outstanding opportunity exists for an experienced and compassionate Registered Nurse specializing in home health care. This role focuses on delivering skilled, patient-centered nursing services within patients homes and is ideal for professionals who are self-directed, organized, and dedicated to maintaining the highest standards of clinical quality and communication.
Contract Details:
- Contract duration: 13 weeks, full-time
- Schedule: Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (generally 35 patient visits daily)
- Field-based, with daily travel to patient homes (mileage reimbursement provided)
- Electronic documentation via Home Care Home Base (HCHB)
- Patient assessment with OASIS tool
Key Responsibilities:
- Complete thorough home health OASIS assessments and develop individualized care plans for each patient
- Deliver a broad scope of nursing interventions including wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient status, recognize changes in condition, and communicate promptly with physicians and interdisciplinary teams
- Educate patients and families on care plans, medications, disease processes, and preventive strategies
- Maintain accurate and timely documentation in accordance with agency and regulatory standards
- Collaborate effectively with team members such as therapists, social workers, and aides to ensure optimal outcomes
- Participate in case conferences, staff meetings, and required continuing education
Qualifications:
- Active RN license in good standing (state of practice)
- At least 1 year recent experience in home health (field experience highly valued)
- Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS
- Strong clinical judgment, communication, and organizational abilities
- Reliable transportation, valid drivers license, and insurance
- Preferred: BSN, familiarity with Medicare/home health regulations, experience managing 2532 points/week
