A rewarding opportunity awaits for an experienced and compassionate Registered Nurse who thrives in the unique environment of home health. Youll deliver skilled care with a personal touch to patients in their own homes, ensuring a focus on quality, safety, and continuity throughout the patient journey. This contract position allows you to work autonomously, leveraging strong clinical skills while developing meaningful relationships with patients and their families.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and create individualized, patient-centered care plans.
• Deliver skilled nursing care, including wound management, IV therapy, medication administration, and chronic disease monitoring.
• Evaluate patient progress, recognize changes in condition, and promptly communicate updates to the care team and physicians.
• Provide patient and caregiver education on disease processes, treatment plans, and preventive care measures.
• Enter and maintain accurate, timely electronic documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team members (therapy, social work, aides) to ensure coordinated care.
• Participate in case conferences and ongoing professional development.
• Adhere to regulatory and professional standards in all aspects of your practice.
• Daily travel to patient homes within the assigned area (mileage reimbursement provided).

Qualifications:

• Active RN license (good standing, state-specific).
• At least 1 year of recent, hands-on Home Health RN experience.
• Proficiency in Home Care Home Base (HCHB) and OASIS.
• Reliable transportation, valid drivers license, and auto insurance.
• Strong assessment skills and comfort working independently.
• BSN preferred.
• Prior experience managing a caseload of 2532 points per week and familiarity with Medicare/home health compliance standards are a plus.
