An outstanding opportunity is available for a skilled and compassionate Home Health Registered Nurse (RN) to deliver high-quality care in a field-based setting. This 13-week contract position is ideal for RNs who excel at working independently and demonstrate clinical expertise, attention to detail, and a commitment to supporting patients in their homes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Provide skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify changes in condition, and communicate with the healthcare team and physicians.
- Educate patients and families regarding care plans, disease processes, and medication regimens.
- Accurately document all care and interventions using Home Care Home Base (HCHB).
- Collaborate with interdisciplinary partners such as physical and occupational therapists, medical social workers, and home health aides.
- Participate in staff meetings, case conferences, and continuing education as appropriate.
- Uphold agency and regulatory guidelines related to documentation and patient care.
Qualifications:
- Active RN license (state of practice) in good standing.
- Minimum of 1 year of recent home health nursing experience (field-based experience highly preferred).
- Proficiency in Home Care Home Base (HCHB) and strong OASIS assessment skills.
- Effective clinical assessment, critical thinking, and time management skills.
- Reliable transportation, valid drivers license, and auto insurance required.
- Ability to work autonomously and efficiently manage a caseload of 2532 points per week.
Preferred:
- BSN degree.
- Experience managing a similar home health caseload.
- Working knowledge of Medicare and home health compliance regulations.
