An exciting opportunity is available for a skilled and compassionate Home Health Registered Nurse. This contract role focuses on providing high-quality, patient-centered care to individuals in their homes. The ideal candidate is an experienced RN with a background in home health, who thrives in an autonomous, field-based environment and prioritizes exceptional clinical care and communication.
Schedule:
- Full-time, 13-week contract
- Monday to Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (typically 35 patient visits daily)
- Field-based: Daily travel to patient homes (mileage reimbursement included)
Primary Responsibilities:
- Perform comprehensive OASIS assessments and establish individualized care plans
- Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management
- Monitor and evaluate patient progress, promptly identifying any changes in condition and communicating with care teams/physicians as needed
- Educate patients and families regarding treatments, medications, and disease management
- Maintain timely, accurate documentation using Home Care Home Base (HCHB)
- Coordinate with an interdisciplinary healthcare team, including therapists and aides, for seamless care delivery
- Participate in case conferences, staff meetings, and ongoing professional development
- Ensure compliance with all regulatory and agency standards
Qualifications:
- Active RN license in good standing
- At least 1 year of recent field-based Home Health RN experience
- Proficient with Home Care Home Base (HCHB) and OASIS assessment tools
- Strong assessment, communication, and time management skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage a flexible caseload
Preferred:
- BSN degree
- Experience managing 2532 points per week
- Familiarity with Medicare and home health regulations
