An opportunity is available for a skilled and compassionate Registered Nurse specializing in Home Health to provide exceptional patient care in a field-based setting. In this role, you will deliver comprehensive nursing services directly in patients homes, facilitating continuity of care and supporting recovery and well-being through individualized care plans.

Responsibilities include conducting thorough OASIS assessments, administering skilled nursing care such as wound management, medication administration, IV therapy, and chronic disease monitoring. You will evaluate patient progress, communicate effectively with the interdisciplinary care team and physicians, and educate patients and families about health conditions and care protocols. Accurate and timely documentation using Home Care Home Base (HCHB) is required, with collaboration among therapists, social workers, and aides to ensure seamless care delivery. Compliance with regulatory standards and active participation in case conferences and continuing education will be integral to your role.

Key Qualifications and Experience:

• Active Registered Nurse license in good standing within the state of practice
• Minimum one year of recent Home Health nursing experience; field experience preferred
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools
• Strong clinical assessment, critical thinking, and independent decision-making skills
• Reliable transportation with a valid drivers license and insurance
• Excellent communication, time management, and organizational abilities
• BSN preferred
• Prior experience managing 2532 points per week and knowledge of Medicare/home health compliance preferred

Contract Details:

• Full-time, 13-week contract
• Monday through Friday, 8:00 AM 5:00 PM
• Caseload of 2532 points per week (approximately 35 patient visits daily)
• Daily travel to patient homes within assigned territory with mileage reimbursement
