An opportunity is available for a skilled and compassionate Registered Nurse with a background in Home Health to deliver patient-focused care in a field-based setting. In this role, youll provide clinical excellence and personalized attention to patients in their homes, making a direct impact on their well-being and outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
As a Home Health RN, you will:
- Conduct comprehensive OASIS assessments and develop individualized patient care plans
- Deliver skilled nursingincluding wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, identify changes in condition, and communicate with physicians and healthcare teams as needed
- Educate patients and families about illness management, medications, and preventive strategies
- Document patient information accurately and promptly using Home Care Home Base (HCHB)
- Collaborate with therapists, social workers, and home health aides for coordinated care
- Participate in case conferences, staff meetings, and ongoing education
- Adhere to all agency and regulatory standards for patient care and documentation
Required qualifications:
- Current, active RN license in the state of practice
- At least 1 year of recent home health (field) experience
- Proficiency in Home Care Home Base (HCHB) and OASIS assessments
- Strong clinical assessment, organizational, and time management skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently in a flexible, field-based environment
- Excellent communication abilities
Preferred:
- BSN degree
- Experience managing a caseload of 2532 points per week
- Knowledge of Medicare and home health compliance
