A rewarding opportunity is available for an experienced and compassionate Home Health Registered Nurse (RN) to provide skilled nursing care in the comfort of patients homes. This role focuses on delivering comprehensive, patient-centered care with an emphasis on clinical excellence and effective communication. Key Responsibilities:
- Conduct thorough OASIS assessments and develop individualized care plans.
- Deliver skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, recognize changes in condition, and communicate promptly with healthcare team members and physicians.
- Educate patients and families about disease management, medications, treatment plans, and preventive care.
- Maintain accurate and timely documentation using Home Care Home Base (HCHB).
- Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides to ensure coordinated care.
- Participate in case conferences, staff meetings, and ongoing continuing education.
- Comply with all regulatory and agency standards for documentation and patient care.
Qualifications:
- Active RN license in good standing within the state of practice.
- At least one year of recent home health nursing experience, with field experience preferred.
- Proficiency in Home Care Home Base (HCHB) and OASIS assessment tool required.
- Strong clinical assessment abilities and critical thinking skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Excellent communication, time management, and organizational skills.
- BSN preferred.
- Experience managing a caseload of 2532 points per week and knowledge of Medicare and home health compliance standards are a plus.
Schedule and Contract Details:
- Full-time, 13-week contract.
- Monday through Friday, 8:00 AM to 5:00 PM.
- Caseload of approximately 35 patient visits per day.
- Field-based work with daily travel within an assigned territory; mileage reimbursement provided.
