Job Description

Seeking an experienced and dedicated Registered Nurse to provide home health care services in a dynamic, field-based setting. In this contract role, you'll deliver skilled nursing care directly to patients homes, focusing on offering high-quality, patient-centered care and fostering positive outcomes. Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Caseload: 2532 points per week (typically 35 patient visits daily)
  • Daily travel within an assigned territory (mileage reimbursement provided)

Key Responsibilities:

  • Complete comprehensive OASIS assessments and develop individualized patient care plans
  • Provide direct skilled nursing services including wound care, IV therapy, medication administration, and chronic disease management
  • Monitor and reassess patient progress, responding promptly to changes in condition and communicating with interdisciplinary team members
  • Educate patients and families about disease processes, treatments, medications, and preventive health practices
  • Ensure timely and accurate documentation using Home Care Home Base (HCHB)
  • Collaborate with therapists, medical social workers, aides, and other healthcare professionals for coordinated care
  • Participate in case conferences, staff meetings, and ongoing education as required
  • Adhere to regulatory requirements and agency protocols for documentation and care delivery

Qualifications:

  • Active, unrestricted RN license
  • At least 1 year of recent home health experience (field-based preferred)
  • Proficient with Home Care Home Base (HCHB) and OASIS
  • Strong assessment, clinical, and communication skills
  • Reliable transportation, valid drivers license, and auto insurance
  • Ability to manage a flexible, independent schedule

Preferred:

  • BSN degree
  • Prior experience managing 2532 points weekly
  • Familiarity with Medicare compliance and home health regulations
