An opportunity is available for an experienced and compassionate Registered Nurse to deliver patient-centered care in a home health setting. This contract position focuses on providing skilled nursing services to individuals in their homes, ensuring consistent care and promoting positive health outcomes. The environment is field-based, requiring independent work within an assigned territory and strong coordination with an interdisciplinary team.

Schedule:

• 13-week contract
• Full-time, Monday through Friday, 8:00 AM 5:00 PM
• Daily travel to patient homes
• Caseload: 2532 points per week, typically 35 visits daily

Core Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized patient care plans
• Deliver skilled nursing care, including wound management, medication administration, and IV therapy
• Monitor and document patient progress in Home Care Home Base (HCHB)
• Communicate effectively with physicians and the broader care team to address changes in patient status
• Educate patients and families on health conditions, medications, and self-care strategies
• Participate in case conferences and ongoing professional development
• Maintain regulatory compliance and thorough documentation

Desired Skills and Experience:

• Active RN license (state of practice) in good standing
• Minimum of one year of recent home health field experience
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance
• Excellent clinical assessment, communication, and organizational skills
• Ability to work independently in a dynamic field environment

Preferred:

• BSN degree
• Familiarity with Medicare and regulatory requirements for home health
• Experience handling caseloads of 2532 points per week

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
