Make an impact in patients lives as a skilled Home Health RN delivering care in a flexible, field-based environment. In this full-time, 13-week contract role, you will independently manage an assigned patient caseload, providing compassionate nursing care and supporting individuals as they recover comfortably at home. The position emphasizes clinical excellence, strong patient communication, and the ability to adapt in a dynamic field setting.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct detailed OASIS assessments and develop individualized care plans.
• Deliver direct skilled nursing services such as wound care, IV therapy, medication administration, and chronic condition management.
• Monitor patient status, detect condition changes, and collaborate with physicians and the interdisciplinary care team.
• Educate patients and families on diagnoses, treatments, medications, and home care protocols.
• Document all visits promptly and accurately using Home Care Home Base (HCHB).
• Coordinate with therapists, medical social workers, and home health aides for integrated patient care.
• Attend case conferences, team meetings, and engage in ongoing professional development.
• Ensure all standards of regulatory compliance and quality are met.

Qualifications:

• Current, active RN license (in good standing).
• Minimum 1 year recent home health experience; field experience preferred.
• Proficient in Home Care Home Base (HCHB) and OASIS assessment.
• Strong skills in assessment, organization, and communication.
• Reliable transportation with valid drivers license and auto insurance.
• Ability to manage a flexible schedule and excel independently.
• Preferred: BSN and experience managing 2532 points per week.
