Job Description
Deliver high-quality, patient-centered care as a Home Health Registered Nurse. In this contract opportunity, youll play a pivotal role in supporting patients in their homes by ensuring consistent, compassionate clinical care and health education. Experienced RNs who excel at independence, efficiency, and strong communication will thrive in this hands-on, field-based position.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract
- Approximately 35 patient visits daily, with a caseload of 2532 points per week
Core Qualifications:
- Active RN license in good standing
- Minimum of 1 year of recent home health experience (field experience highly valued)
- Proficiency with Home Care Home Base (HCHB) electronic documentation
- OASIS assessment experience
- Reliable transportation, valid drivers license, and auto insurance
- Solid assessment, critical thinking, organization, and communication skills
- Ability to work autonomously and manage a flexible field-based schedule
Preferred Experience:
- Bachelor of Science in Nursing (BSN)
- Previous management of 2532 points per week
- Knowledge of Medicare and home health compliance standards
Daily Responsibilities:
- Complete OASIS assessments and create effective patient care plans
- Deliver skilled nursing (wound care, medication administration, IV therapy, chronic disease management) in patient homes
- Monitor and report any changes in patient conditions to the care team and physicians
- Educate patients and families on health management, medications, and prevention
- Accurately document care in HCHB system and participate in case conferences, meetings, and ongoing education
- Collaborate with a multidisciplinary team to ensure comprehensive care
