Bring your clinical expertise and compassionate care to a rewarding role as a Home Health Registered Nurse. In this position, you will make a direct impact by delivering skilled nursing services to patients in their own homes, providing both high-quality care and comfort to individuals managing acute and chronic health needs. This full-time, 13-week contract offers the opportunity to work autonomously, collaborate with an interdisciplinary care team, and serve as a valuable resource for both patients and their families.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points/week (typically 35 patient visits per day)
- Field-based, daily travel within assigned territory (mileage reimbursement included)
Qualifications:
- Active RN License (in good standing)
- Minimum of 1 year recent, hands-on Home Health RN experience
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment tool
- Excellent clinical assessment skills, critical thinking, and patient communication
- Valid drivers license, reliable transportation, and current auto insurance
- Ability to work independently and maintain timely, accurate documentation
- BSN preferred
- Familiarity with Medicare and home health compliance is a plus
Key Responsibilities:
- Conduct OASIS assessments and develop individualized care plans
- Provide skilled nursing (wound care, medication administration, IV therapy, disease management)
- Monitor and evaluate patient progress, updating care teams as needed
- Educate patients and families about care plans and health management
- Maintain documentation in HCHB and adhere to regulatory standards
- Collaborate with therapists, social workers, and aides to coordinate holistic care
- Participate in case conferences and ongoing professional development
