An exciting opportunity is available for an experienced and compassionate Registered Nurse specializing in home health to provide high-quality care to patients in a supportive field-based environment. This full-time role is structured as a 13-week contract and is well-suited for independent, organized professionals who thrive on promoting patient well-being directly in their homes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Field-based; daily travel to patient homes within an assigned territory
- Caseload: 2532 points per week (typically 35 patient visits daily)
Qualifications:
- Active RN license (in good standing) in the state of practice
- Minimum of 1 year of recent home health nursing experience (field experience strongly preferred)
- Proficiency with Home Care Home Base (HCHB) electronic documentation
- Direct OASIS assessment experience
- Reliable transportation, valid drivers license, and active auto insurance
- Solid clinical assessment, time management, and organizational skills
- Excellent communication and ability to work autonomously
- BSN preferred
- Experience managing 2532 home health points weekly and knowledge of Medicare/home health compliance are beneficial
Role Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing care: wound care, medication administration, IV therapy, chronic disease management
- Monitor patient progress, identify and communicate changes promptly, and ensure timely documentation
- Educate patients and families on diagnoses, treatment plans, medications, and preventive care
- Work closely with an interdisciplinary teamincluding therapists and home health aidesfor coordinated care
- Attend case conferences, engage in staff meetings, and participate in ongoing education
- Uphold regulatory and agency care delivery standards
