Job Description

Experienced and compassionate Registered Nurses are sought for a home health assignment providing vital care to patients in their homes. This role focuses on delivering high-quality, patient-centered nursing services, ensuring continuity and excellence with every interaction. Candidates should thrive in an autonomous field-based setting, remain organized under varying caseloads, and communicate effectively to support positive outcomes for patients and families.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct detailed OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing interventions (wound care, medication administration, IV therapy, chronic disease management).
  • Monitor patient progress, document changes, and collaborate promptly with the multidisciplinary care team.
  • Educate patients and families on clinical treatments, preventive care, and health conditions.
  • Accurately document within Home Care Home Base (HCHB) electronic system.
  • Assist in case conferences, attend staff meetings, and participate in continuing education.
  • Adhere strictly to regulatory and agency standards for quality care and documentation.

Qualifications:

  • Active RN license (in good standing) for the state of practice.
  • At least one year of recent field-based home health experience.
  • Proficiency in Home Care Home Base (HCHB) and OASIS required.
  • Strong clinical assessment, communication, and organizational skills.
  • Reliable transportation, current drivers license, and valid auto insurance.
  • Ability to work independently and manage a variable caseload (2532 points per week; 35 visits daily).
  • BSN preferred; experience managing similar caseloads and knowledge of Medicare/home health compliance a plus.

Contract Details:

  • Full-time, 13-week contract.
  • Daily travel within an assigned territory; mileage reimbursement provided.
