A skilled and compassionate Registered Nurse is needed to provide home-based care to patients, ensuring excellence in clinical delivery and patient communication. This is a full-time, 13-week contract role, offering the chance to play a key role in patient health management in a supportive environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Role Highlights:

• Field-based position performing daily travel to patient homes (mileage reimbursement provided)
• Caseload assignment of 2532 points per week (approximately 35 patient visits per day)
• Use of Home Care Home Base (HCHB) for electronic documentation and OASIS as the assessment tool

Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans.
• Deliver a range of skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, promptly communicate changes with physicians and the care team, and adjust care plans as needed.
• Educate patients and families on treatment regimens, medications, and preventive strategies.
• Maintain accurate, timely clinical documentation and ensure regulatory compliance.
• Collaborate closely with therapists, social workers, and home health aides to coordinate patient services.
• Participate in case conferences, staff meetings, and ongoing education.

Requirements:

• Active RN license in good standing
• Minimum of 1 year of recent Home Health RN experience (field work preferred)
• Proficient in Home Care Home Base (HCHB) and OASIS
• Reliable transportation, valid drivers license, and car insurance
• Excellent clinical, organizational, and communication skills
• Ability to manage a flexible schedule and work independently

Preferred:

• BSN degree
• Experience managing 2532 points per week
• Familiarity with Medicare and home health regulations
