Deliver excellent, patient-centered care as a Home Health Registered Nurse, providing essential healthcare services directly in patients homes. This full-time, 13-week contract role is ideal for a skilled RN who thrives in an autonomous, field-based environment and is passionate about supporting patients recovery and independence.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Role Details:

• Caseload of 2532 points per week (typically 35 home visits daily)
• Field-based position (daily travel required; mileage reimbursement provided)
• Utilizes Home Care Home Base (HCHB) for electronic documentation
• OASIS assessment responsibilities

Key Responsibilities:

• Conduct comprehensive OASIS assessments, developing and managing individualized care plans
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, observe and promptly communicate any changes in condition
• Instruct patients and their families about disease processes, medications, and health maintenance
• Maintain timely, accurate patient records in HCHB
• Collaborate with physical/occupational therapists, social workers, and home health aides to ensure seamless care
• Participate in case conferences, staff meetings, and maintain ongoing professional education
• Uphold regulatory standards and agency protocols

Qualifications:

• Active RN license in good standing in the state of practice
• At least 1 year of recent home health (field-based) RN experience
• Proficient with Home Care Home Base (HCHB) and OASIS assessments
• Excellent assessment, critical thinking, and communication skills
• Reliable transportation, valid drivers license, and current auto insurance
• Capable of independent work and strong time management

Preferred credentials include a BSN, prior experience managing a 2532 point caseload, and familiarity with Medicare/home health compliance.
