A dynamic opportunity is available for an experienced and compassionate Registered Nurse to provide skilled home health care for patients in a supportive, field-based environment. This full-time, 13-week contract position centers on delivering high-quality, patient-centered care that promotes independence and positive outcomes.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM

Role Highlights:

• Conduct OASIS assessments and create tailored care plans for each patient.
• Deliver skilled nursing services, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, perform ongoing re-assessments, and coordinate with physicians and interdisciplinary teams.
• Educate patients and families on managing conditions, medications, and self-care practices.
• Maintain thorough, accurate documentation using Home Care Home Base (HCHB).
• Attend case conferences, team meetings, and participate in continuing education opportunities.
• Daily travel within your assigned territory (mileage reimbursement included).
• Typical caseload: 2532 points per week (generally 35 patient visits daily).

Qualifications:

• Active, unencumbered RN license in the state of practice
• Minimum 1 year of recent home health nursing experience (field preferred)
• Documented proficiency in Home Care Home Base and OASIS
• Reliable transportation, valid drivers license, and current auto insurance
• Strong clinical judgement, communication, and organizational skills
• Ability to work independently and effectively manage a flexible schedule
• BSN and knowledge of Medicare/home health compliance preferred
