Job Description

An experienced and compassionate Home Health Registered Nurse (RN) is needed to provide skilled nursing care directly in patients homes. This full-time, 13-week contract role involves delivering patient-centered care with a strong focus on clinical excellence and effective communication. The successful candidate will work independently, managing multiple patient visits daily while ensuring quality, continuity, and positive health outcomes.

Key Responsibilities:

  • Perform comprehensive OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, detect condition changes, and communicate promptly with care teams and physicians.
  • Educate patients and families on disease management, treatment plans, medications, and preventive care.
  • Maintain thorough and timely documentation using Home Care Home Base (HCHB) software.
  • Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides for coordinated care.
  • Participate in case conferences, staff meetings, and continuing education.
  • Adhere to regulatory and agency standards for documentation and care delivery.

Qualifications:

  • Active RN license in good standing within the state.
  • Minimum 1 year recent home health experience, preferably field-based.
  • Proficient in HCHB electronic documentation system.
  • Experience with OASIS assessment tool required.
  • Strong clinical assessment, critical thinking, and communication skills.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to work independently managing a flexible field schedule.

Preferred:

  • Bachelor of Science in Nursing (BSN).
  • Experience managing 2532 points per week caseload.
  • Knowledge of Medicare and home health compliance standards.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM.
  • Typical caseload includes 35 patient visits daily (2532 points weekly).
  • Daily travel to patient homes within assigned territory with mileage reimbursement.
