Job Description

Delivering compassionate end-of-life care takes skill, empathy, and dedication. We are seeking an experienced Registered Nurse with a hospice background to provide patient-focused care in home and residential settings. This contract position offers an opportunity to make a meaningful difference, working closely with both patients and interdisciplinary team members.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM
  • Occasional weekend or on-call rotation may be required
  • Expectation of 45 patient visits daily

Key Responsibilities:

  • Perform comprehensive nursing assessments and develop individualized care plans for hospice patients
  • Deliver skilled nursing care, including symptom management, pain control, and comfort interventions
  • Administer medications and treatments according to physician orders and established hospice protocols
  • Monitor patient condition changes and promptly communicate with the care team and medical director
  • Educate patients, families, and caregivers about disease progression, comfort measures, and medication administration
  • Collaborate with social workers, chaplains, and CNAs to address holistic needs
  • Maintain accurate, timely documentation in Home Care Home Base (HCHB)
  • Participate in team meetings and quality improvement activities
  • Ensure compliance with organizational and regulatory guidelines

Required Qualifications:

  • Active RN license in the state of practice, in good standing
  • Minimum of 1 year of recent hospice nursing experience
  • Proficiency in Home Care Home Base (HCHB)
  • Exceptional clinical assessment and communication skills
  • Strong understanding of end-of-life care principles
  • Reliable vehicle, valid drivers license, and current auto insurance
  • Ability to work independently and manage a field-based schedule
  • Compassionate and emotionally resilient

Preferred:

  • BSN degree
  • Experience in palliative care
  • Familiarity with Medicare hospice regulations
