An exciting opportunity is available for a dedicated and skilled Registered Nurse specializing in home health care. This position focuses on supporting patients in their own homes, ensuring continuity of care, clinical excellence, and compassionate communication. The ideal candidate will thrive in an autonomous environment, utilizing strong organizational skills and attention to detail to deliver high-quality nursing services.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Perform comprehensive OASIS assessments and design individualized care plans.
• Deliver skilled nursing care, such as wound treatment, medication administration, IV therapy, and chronic disease management.
• Track patient progress, relay health changes promptly to physicians and interdisciplinary teams.
• Provide education to patients and families on disease management, medications, and preventative care.
• Accurately document all activities in Home Care Home Base (HCHB).
• Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides.
• Participate in case conferences, staff meetings, and ongoing education.
• Ensure compliance with agency and regulatory clinical standards.

Qualifications:

• Active RN license in good standing in the state of practice.
• At least 1 year of recent home health nursing experience (field experience preferred).
• Proficiency in Home Care Home Base (HCHB) and familiarity with OASIS required.
• Excellent communication, time management, and critical thinking skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to manage a flexible, field-based schedule independently.
• BSN preferred and prior experience maintaining caseloads of 2532 points per week a plus.
• Knowledge of Medicare and home health compliance standards is beneficial.
