We are searching for a dedicated and experienced Home Health Registered Nurse (RN) to deliver compassionate, high-quality care to individuals in need. In this key field-based role, you will draw on your clinical skills to support patients health, independence, and recovery.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Daily travel between patient homes within a designated territory (mileage reimbursement provided)
• Caseload: 2532 points per week (typically 35 patient visits per day)

Qualifications:

• Active RN license in good standing
• Minimum one year of recent home health nursing experience
• OASIS assessment experience and proficiency with Home Care Home Base (HCHB) required
• Strong clinical assessment, organizational, and communication skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work autonomously and manage a flexible field-based schedule
• BSN preferred
• Experience managing 2532 points per week is a plus
• Familiarity with Medicare and home health compliance standards is desirable

Key Responsibilities:

• Complete comprehensive OASIS assessments and design effective care plans
• Deliver skilled nursing interventions: wound care, IV therapy, medication administration, and disease management
• Monitor patient progress, communicate changes to the care team and physicians
• Provide patient/family education about treatment, medications, and health management
• Maintain detailed and timely documentation using HCHB
• Collaborate with therapists, social workers, and aides to coordinate care
• Engage in case conferences, team meetings, and ongoing professional development
