Job Description
Deliver expert and compassionate end-of-life care as a Hospice Registered Nurse supporting patients in their homes and residential settings. This vital contract role is dedicated to providing comfort, dignity, and holistic support to individuals and families navigating terminal illness.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call rotation as needed
Key Responsibilities:
- Conduct comprehensive assessments and develop individualized hospice care plans
- Deliver skilled nursing care focused on symptom and pain management, always prioritizing patient comfort
- Administer medications, treatments, and interventions as prescribed
- Monitor changes in patient condition, reporting promptly to the medical director and care team
- Educate patients, families, and caregivers on disease progression, comfort measures, and medication administration
- Collaborate with a multidisciplinary teamincluding social workers, chaplains, aides, and volunteersto meet patients physical, emotional, and spiritual needs
- Complete accurate and timely documentation in the Home Care Home Base (HCHB) system
- Participate in case conferences, team meetings, and quality improvement initiatives
- Ensure adherence to hospice policies, procedures, and regulatory standards
Qualifications:
- Active, unencumbered RN license in the state of practice
- At least 1 year of recent experience in hospice nursing
- Proficiency using Home Care Home Base (HCHB) is essential
- Excellent clinical assessment and communication abilities
- In-depth knowledge of pain management and end-of-life care principles
- Reliable transportation, valid drivers license, and auto insurance
- Self-motivated with a compassionate, resilient mindset
Preferred:
- BSN
- Previous palliative or end-of-life care experience
- Familiarity with Medicare hospice regulations and documentation standards
