Job Description
Deliver skilled and compassionate nursing care directly to patients homes in this contract role. This position focuses on providing high-quality, patient-centered care, independently managing a defined caseload, and collaborating with interdisciplinary healthcare teams for optimal outcomes.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans
- Provide skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient conditions, evaluate progress, and communicate changes to the care team and physicians
- Educate patients and families on treatment, medications, and preventive strategies
- Accurately document visits and care in Home Care Home Base (HCHB)
- Coordinate with therapists, social workers, and home health aides to ensure thorough, cohesive care
- Participate in team meetings, case conferences, and continuing education
- Ensure adherence to regulatory standards and best practices in home health nursing
Qualifications:
- Current, active RN license (in good standing)
- At least one year of recent home health nursing experience (field experience strongly preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS assessments required
- Strong assessment and critical thinking abilities
- Self-motivated with excellent organizational, communication, and time management skills
- Reliable transportation, valid drivers license, and current auto insurance
- BSN and experience managing a caseload of 2532 points per week preferred
- Knowledge of Medicare and home health compliance standards is valued
