Deliver skilled, patient-centered nursing care directly to individuals in their homes as a Home Health Registered Nurse. This field-based position ensures continuity of care while supporting patients health, independence, and quality of life through expert clinical oversight.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

In this 13-week contract role, responsibilities include:

• Conducting comprehensive OASIS assessments and utilizing findings to design tailored care plans
• Delivering skilled nursing tasks such as wound care, medication administration, IV therapy, and chronic disease management
• Closely monitoring patient status, promptly responding to changes, and updating the interdisciplinary team and physicians as needed
• Providing clear education to patients and families on treatment plans, medications, and preventive care
• Documenting care accurately using the Home Care Home Base (HCHB) system
• Collaborating with other clinicianssuch as physical therapists, occupational therapists, and social workersto ensure a fully coordinated approach
• Participating in case conferences, meetings, and ongoing training to maintain clinical excellence
• Traveling daily to patient homes within the assigned territory (mileage reimbursement available)

Qualifications:

• Current, unrestricted RN license in the state
• At least 1 year of recent, hands-on Home Health nursing experience
• Experienced with OASIS assessment and HCHB electronic documentation
• Outstanding clinical judgment, critical thinking, and organizational skills
• Strong independent work ethic; reliable transportation, valid drivers license, and auto insurance
• Exceptional written and verbal communication

Preferred candidates will have a BSN, experience handling a caseload of 2532 points per week, and a strong understanding of Medicare/home health compliance standards.

$1,770 - $1,873/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
