An exciting opportunity is available for a skilled and compassionate Registered Nurse with strong home health experience. This full-time, 13-week contract role is ideal for a self-motivated RN who excels at providing high-quality, individualized care within patients homes. Youll play a pivotal role in patient outcomes by collaborating with interdisciplinary teams and supporting patients and families throughout their care journey.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Responsibilities:
- Conduct comprehensive OASIS assessments to develop tailored care plans.
- Deliver skilled nursing services such as wound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient status, respond to changes in condition, and ensure timely communication with healthcare providers.
- Educate patients and families on disease management, medications, and preventive techniques.
- Document thoroughly and promptly using Home Care Home Base (HCHB).
- Work collaboratively with therapists, medical social workers, and home health aides to coordinate care.
- Attend case conferences, staff meetings, and participate in ongoing education.
- Uphold regulatory and agency standards for documentation and care delivery.
Required Qualifications:
- Active and unrestricted RN license.
- At least 1 year of recent field experience in home health nursing.
- Proficiency with Home Care Home Base (HCHB) and OASIS.
- Excellent assessment, critical thinking, and communication skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to work independently and manage a caseload of 2532 points weekly.
Preferred:
- Bachelors degree in Nursing (BSN).
- Familiarity with Medicare and home health compliance regulations.
- Experience with similar caseload demands.
