Job Description

Elevate your nursing career as a Home Health Registered Nurse, providing patient-centered care in a dynamic, field-based position. As an integral part of the interdisciplinary healthcare team, you'll deliver skilled nursing services to clients in their homes, fostering independence and improved health outcomes through compassionate, evidence-based care.

Schedule:

  • Monday Friday
  • 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct comprehensive OASIS assessments to develop individualized care plans.
  • Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management.
  • Monitor patient progress, promptly report changes in condition, and coordinate with physicians and the care team.
  • Educate patients and families regarding disease management, medications, and home safety.
  • Maintain accurate documentation using the Home Care Home Base (HCHB) electronic system.
  • Collaborate with physical therapists, occupational therapists, social workers, and home health aides to ensure seamless, coordinated care.
  • Attend case conferences and staff meetings and participate in ongoing education.
  • Adhere strictly to regulatory, compliance, and agency standards.

Qualifications:

  • Current RN license in good standing (state of practice)
  • Minimum 1 year of recent Home Health experiencefield experience highly valued
  • Proficiency in Home Care Home Base (HCHB) and OASIS assessment required
  • Reliable transportation, valid drivers license, and proof of auto insurance
  • Strong clinical judgment, organization, and communication skills
  • Ability to work independently, navigating a flexible field-based role

Preferred:

  • Bachelors in Nursing (BSN)
  • Experience managing a caseload of 2532 points per week
  • Familiarity with Medicare and home health compliance
