Deliver essential care where it matters most as a Home Health Registered Nurse supporting patients in their residences. Step into a dynamic field-based role providing skilled nursing, compassionate support, and continuity of careall while collaborating with a dedicated interdisciplinary healthcare team.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop patient-specific care plans.
• Deliver skilled nursingsuch as wound care, medication management, IV therapy, and chronic disease supportin patient homes.
• Evaluate patient progress, flag changes in condition, and maintain continuous communication with physicians and healthcare colleagues.
• Educate patients and family members about disease process, medications, and preventive strategies.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Partner with physical/occupational therapists, social workers, and home health aides to provide coordinated, holistic care.
• Participate in staff meetings, ongoing education, and case conferences.
• Practice in accordance with regulatory and agency guidelines for home health.

Qualifications:

• Active RN license in good standing for the state of practice
• Minimum 1 year of recent Home Health nursing experience (field experience highly preferred)
• Advanced proficiency with Home Care Home Base (HCHB) and OASIS required
• Excellent clinical assessment, communication, and organizational skills
• Strong ability to work independently and manage a flexible field schedule
• Reliable transportation, valid drivers license, and proof of auto insurance

Preferred:

• Bachelors degree in Nursing (BSN)
• Experience managing a caseload of 2532 points per week
• Familiarity with Medicare/home health compliance standards
