Deliver essential care where it matters most as a Home Health Registered Nurse supporting patients in their residences. Step into a dynamic field-based role providing skilled nursing, compassionate support, and continuity of careall while collaborating with a dedicated interdisciplinary healthcare team.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop patient-specific care plans.
- Deliver skilled nursingsuch as wound care, medication management, IV therapy, and chronic disease supportin patient homes.
- Evaluate patient progress, flag changes in condition, and maintain continuous communication with physicians and healthcare colleagues.
- Educate patients and family members about disease process, medications, and preventive strategies.
- Maintain accurate, timely documentation using Home Care Home Base (HCHB).
- Partner with physical/occupational therapists, social workers, and home health aides to provide coordinated, holistic care.
- Participate in staff meetings, ongoing education, and case conferences.
- Practice in accordance with regulatory and agency guidelines for home health.
Qualifications:
- Active RN license in good standing for the state of practice
- Minimum 1 year of recent Home Health nursing experience (field experience highly preferred)
- Advanced proficiency with Home Care Home Base (HCHB) and OASIS required
- Excellent clinical assessment, communication, and organizational skills
- Strong ability to work independently and manage a flexible field schedule
- Reliable transportation, valid drivers license, and proof of auto insurance
Preferred:
- Bachelors degree in Nursing (BSN)
- Experience managing a caseload of 2532 points per week
- Familiarity with Medicare/home health compliance standards
